Documentation showing legal notice of eviction or utility shut-off notice (gas, electric, water) is required.

I've used this service before.and received: 250. The staff was unfriendly. I waited more than 24 hours township refuse to give an appeal form by saying we don't do appeal forms but paper i got from them says that i need to put it in writing couldn't because they say we don't have forms and kept leaving me at window trying to act like i wasn't there
