Rent or mortgage paymentsUtility billsOne phone billCar payments, emergency car repairs, and insurance for one carFood and gas gift cardsCAN is not able to pay the following bills:
No medical or pharmaceutical billsNo credit card billsNo tax billsNo entertainment billsNo condo or neighborhood association feesNo home or health insuranceIn order to be eligible, applicants must:
Currently be in treatment for either chemo or radiation OR in the care of HospiceBe able to provide proof of residency in Collier or Lee Counties for a minimum of one year (bring copy of lease, tax return or other validation)Meet certain financial income requirements (bring paycheck stubs, SSDI letter, or other proof). Please note that revenue from all family members must be included in household incomeProvide photo identification (such as a drivers license or passport)Fill out the application for Beneficial AidHave your physician fill out his/her portion of the Physicians Verification FormPlease contact them directly to obtain more information or to schedule an appointment.
