Consists of three components: Emergency Services, Intervention Services, and Self-Sufficiency Services. Examples of services provided are: rental/mortgage assistance, utility assistance, food assistance and transportation. A comprehensive assesment is completed. All services are provided in the context of an individual plan which is developed between the applicant and the case manager based on identified service needs. On-going case management is provided to facilitate and support the individual/family ability to gain or regain self sufficiency. In addition to these services the Division has statutory requirements to provide certain services under the Health Care Responsibility Act (HCRA). The HCRA program mandates county payment for emergency hospital care provided in another Florida County when the person resides in Palm Beach County. Eligibility for this program is based upon income guidelines.
